اثر مداخله تمرین ورزشی ترکیبی بر شاخص اندوتلین-۱ در رابطه با تغییرات استرس روانی و همودینامیکی در زنان مبتلا به مولتیپل اسکلروزیس

چکیده
هدف: با وجود ارتباط بین سفتی شریانی با تخریب میلین، درک اختلال عروقی و نقش مداخلات غیردارویی مانند فعالیت ورزشی در عملکرد عروقی بیماران مولتیپل اسکلروزیس (MS) همچنان محدود است؛ بنابراین در مطالعه حاضر به بررسی اثر مداخله تمرین ورزشی ترکیبی بر شاخص اختلال عملکرد اندوتلیال (اندوتلین-۱) در رابطه با تغییرات استرس روانی و همودینامیکی در زنان مبتلا به MS پرداخته شد.
مواد و روشها: در این پژوهش تجربی، 30 زن مبتلا به MS (سن 9/3±8/36 سال و وزن 1/11±72 کیلوگرم) به طور تصادفی و مساوی به دو گروه تجربی و کنترل تقسیم شدند. گروه تجربی 12 هفته در برنامه تمرینی ترکیبی (تمرین هوازی (11-13 RPE)، مقاومتی (باندکشی، 1-3 ست 8 تا 15 تکراری)، تعادلی و تنفسی-کششی حداقل 60 دقیقه، سه جلسه در هفته) شرکت کردند. قبل و بعد از دوره مداخله سطح سرمی اندوتلین-1 ، فشارخون، ضربان قلب استراحتی، اضطراب و عملکرد حرکتی ارزیابی شد. تحلیل داده‌ها به کمک آزمون آنکوا صورت گرفت.
یافتهها: تمرینات ورزشی، سطوح اندوتلین-1 و اضطراب را در گروه تمرینی در مقایسه با گروه کنترل کاهش داده و عملکرد حرکتی را بهبود دادند (05/0<P). همچنین ارتباط معناداری بین کاهش سطح اضطراب و اندوتلین-1  مشاهده شد (754/0=r،  003/0=P). با وجود بهبود شاخص‌های همودینامیکی، ارتباط معناداری بین کاهش آن‌ها و کاهش اندوتلین-1 وجود نداشت (05/0<P).   
نتیجهگیری: تمرین ترکیبی موجب کاهش سطوح اندوتلین-1 در بیماران مبتلا به MS می‌شود و این کاهش بیش از آنکه با تغییرات فشارخون مرتبط باشد، ناشی از کاهش اضطراب روانی است. نتایج پژوهش بر نقش مداخلات تمرینی ترکیبی در بهبود هم‌زمان ابعاد فیزیولوژیک، روان‌شناختی و عملکرد حرکتی بیماران مبتلا به MS تأکید می‌کند.
